Financial Policies and Information Collected: Because we provide elective cosmetic procedures, the care provided at Advanced Skin and Vein Care Centers is not covered by any medical insurance programs, and we do not participate in any such plans. Some of the content and medical spa services available in our location and on our website require registration. We may collect information (including but not limited to: name, phone number, mailing address, email address, etc). We may use this information to contact you if the need arises and to send you information about our company and promotional material. You have the right to opt out of receiving such mailings. Your credit card information may be stored by our client management partner. We reserve the right to change our client management partner or our payment processing partner at any time and without notice. If this concerns, you check back regularly for changes. We reserve the right to use the stored credit card information to pay for continuing or recurring memberships, no show/late arrival fees, and outstanding balances on the client’s account. Payment Options: Payment for all medical spa procedures is due at time of treatment. All specially packaged treatments, grouped, or package pricing is due at first scheduled appointment. We accept cash, personal checks, Visa, MasterCard, American Express, and Discover, Flex spending or health savings cards are not accepted. Care Credit is not accepted for ANY INJECTABLES or THREADS.
Visit Cancellation Policy: All clients are required to pay a $75 deposit at the time of booking the first appointment and $50 for follow up appointments, the deposit will be credited to the services rendered or refunded if no services are provided, in the event of a no show or cancellation less than 48 hours the deposit will NOT be refunded.
Refund-Services: At Advanced Skin & Vein Care Centers we work with each client to discuss treatment objectives and review likely outcomes, benefits and risks associated with each treatment. We offer individual treatment as well as significantly discounted treatment package options so each client may choose the approach that is best suited for their needs and budget. Once services are purchased, they will not be refunded, however, to ensure our clients always receive the greatest experience at Advanced Skin & Vein Care Centers unused service values (cash equivalent for the remaining amount of a treatment package) can be applied to any other service at Advanced Skin & Vein Care Centers. For Skin Care Products, all sales are final, however, should you have a skin reaction to one of the products, it can be returned for a full refund within 7 days of purchase. All injectable treatment sales (such as, but not limited to Botox, Dysport, dermal fillers, laser treatments, facials, lash services, or chemical peels) are final; refunds or credits cannot be offered once treatment is completed. We will be honest in all our dealings with you. Aesthetics is not an exact science and how you may respond to a given treatment will vary from person to person. It is virtually impossible to predict results and therefore payments made for services are for treatments to be performed -- not for a specific result. However, we always strive to achieve the absolute best result that we can for you.
Revisional Treatment or Treatment of Complications: The practice of medicine and surgery is not an exact science, and cosmetics are the practice of medicine. Although good results are anticipated, there can be NO GUARANTEE OR WARRANTY, expressed or implied by anyone as to the actual results you may get. Occasionally additional treatments and/or treatment for problems or complications may be required. These could result in additional charges for which you may be responsible. Your insurance, if you have it, may or may not cover expenses related to actual complications or other medically related problems arising out of treatment at Advance Skin and Vein Care Centers. Personal belongings: Personal belongings are the full responsibility of the spa guest and should be kept in your possession at all times. Advanced Skin & Vein Care Centers is not responsible for lost or damaged items.
Contact Policy: I understand that the only official means of communication with my physician is during the actual office visit. After normal business hours, Advanced Skin & Vein Care Centers will have a physician on call for emergency calls only via local Emergency Departments. Because my physician is busy caring for patients either in the hospital or in the office for the entire day, any other phone calls may be returned after normal business hours, at the sole discretion of the treating physician.
